APPLICATION FORM IN ACCORDANCE WITH ARTICLES 11 AND 13 OF THE LAW ON THE PROTECTION OF PERSONAL DATA

Personal data owners, defined as data subjects in the Law No. 6698 on the Protection of Personal Data (hereinafter referred to as "Applicant"), are granted the right to make certain requests regarding the processing of their personal data in Article 11 of the Law.

In accordance with the first paragraph of Article 13 of the Law, applications regarding these rights must be submitted to our company, which is the data controller, in writing or through other methods determined by the Personal Data Protection Board.

Your application and the request contained therein will be processed by YAKE, the data controller, free of charge as soon as possible and no later than thirty days, depending on the nature of the request. However, if the process requires additional costs, a fee may be charged according to the tariff determined by the Board. Your request will be accepted or rejected with justification within this period, and the response will be communicated to you in writing or electronically.

According to Article 7 titled "Fee" of the Communiqué on the Procedures and Principles for Applications to the Data Controller, "(1) If the response to the data subject's application is to be given in writing, no fee will be charged for up to ten pages. A processing fee of 1 Turkish Lira may be charged for each page exceeding ten pages. (2) If the response to the application is given on a recording medium such as a CD or flash drive, the fee that may be requested by the data controller cannot exceed the cost of the recording medium."
